Middle is a bit of a muddle, and I'd suggest not using it. Use clef=treble3 and the like to move the clef and use octave to shift octaves to avoid commas on input. One solution is to add the line

P:AAAA

in the header and then define

P:A

in the body.

That will get the right playback, but unfortunately will not match the old "four dot" notation method.
